Supportive Housing Case Manager (Full Time)

Primary responsibilities:

1) Outreach and engagement with homeless and formerly homeless men to connect them with housing and resources to maintain housing.

2) Coordination of services, support team meetings, and advocacy to assist residents with individual goals.

3) Crisis prevention and intervention for interpersonal and community conflict, health and mental health emergencies, and substance abuse relapse.

4) Facilitation of community building through social and informational group activities onsite, supported interaction, and opportunities to become involved in activities offsite.

Clinical Assistant (PRN/evenings, weekends)

Primary responsibilities:

1) Provide case management follow-up and community-building activities as the sole staff member onsite each shift.

2) As the first-responder onsite, provide crisis prevention and intervention for resident emergencies.

3) Monitor for safety and security issues and respond with support for residents as well as coordination with property staff for property emergencies.
